    I think Swiss cheese holes in liver is fine.  I don't know how it forms the holes though.

I've been doing a bit of reading, and I think the holes might simply be where major blood vessels passed through the liver.  (Maybe the vessels themselves are pulled out by the butcher or as the liver is extracted from the body.)
